Message:	NAME: COUNTY: CITY or VILLAGE: STREET and NUMBER: APPROX DATE and PLACE SIGNED: Dear Sir or Madam, Recently I was approached by a petiton circulator and was asked to sign a petiton regarding porn stores and/or strip bars. I signed it based on what the circulator told me but have recently been informed that the circulator was not truthful about the petitioner's real intent. The Ohio Revised Code Sec. 3599.14 says, "No person shall knowingly, directly or indirectly...misrepresent the contents, purpose, or effect of the petition." Therefore, pursuant to Ohio Revised Code Sec. 3501.38(G), I am writing to request that the circulator REMOVE my name from the petition BEFORE it is filed in a public office or submitted to the Ohio Secretary of State or my County Board of Elections for validation. Request is hereby made for a reply to confirm your receipt of my request.
